I just screwed over ~30 people in the MMORPG I play. I uploaded what I thought to be a fixed DLL file for the error everyone seems to be encountering, but it turns out it was corrupted. LUCKILY, I had a backup I could upload, but I have this nagging feeling that at least a few people are going to redownload the whole game, rather than seeing my updated post. (i'm amazed that 30 people managed to see the download location in the short amount of time it was there -- There was only a 15 minute gap)

Shortly after getting my first ever PC, I started writing apps*. I managed to completely hose a friend's Windows (98) installation by replacing a system DLL file (ole32.dll, or something) that required installing the OS again.

Oops.

* Was previously doing stuff on the Amiga which didn't really seem to care what you replaced. Yeah, Windows kinda doesn't like that...
